By registering with with North Sydney United Football Club, the player, and in the case of a registering a minor, both the player and their parents and/or guardians agree to the following membership terms and conditions of the Club;
1. I request that North Sydney United Football Club accept this registration and membership for Season 2011.
2. I declare that the information contained in my registration form is true and accurate.
3. I agree to abide by the rules, policies, statutes and regulations of North Sydney United Football Club, Kuring-gai District Soccer Association (KDSA), Football NSW, Football Federation Australia, and FIFA.
4. I agree to pay all fees owing to North Sydney United Football Club prior to playing in the 2011 competition.
5. I declare that I do not owe any moneys to and am not registered to play with any other soccer club in New South Wales and do not unlawfully possess any equipment belonging to any other club.

7. I declare that I have received and reviewed the Football NSW Accident Support brochure for 2011 available on Football NSW website.
8. I acknowledge that the Club and KDSA has a partnership with Sydney FC Pty Ltd to allow Sydney FC to provide special offers to Club members, and I permit the Club or KDSA to pass on my details to Sydney FC for this purpose. If I am a registered player 12 years and younger, I confirm I would like to receive the FREE Sydney FC Junior Blues Membership which entitles me to FREE entrance to all Sydney FC home games in the 2011/12 Hyundai A-League season. If I am a registered player 13 years and older, I confirm I would like to receive a FREE ticket to a Sydney FC home game in the 2011/12 Hyundai A-League season. Should I not wish to receive such offers I will notify the club via email to the Club President at president@northsydneyunited.com.au
9. I acknowledge that the Club has a partnership with AC Milan Soccer School to allow AC Milan Soccer School to provide special offers to Club members, and I permit the Club to pass on my details to AC Milan Soccer School for this purpose. Should I not wish to receive such offers I will notify the club via email to the Club President at president@northsydneyunited.com.au
10. I understand that the club is run entirely by volunteers and cannot operate without the assistance of players and parents of players each week in fulfilling the many and varied duties required. For a team to be registered the following positions must be filled by players or parents of players:
a. Team Manager;
b. Team Coach;
c. Team Referee (one or more, as required per competition rules);
d. Team Executive/Committee Member; and
e. Team Canteen Volunteer (when playing at Cammeray or Tunks Park only)
Failure of a team to provide all of the above can result in the team not being registered to play.
REFUND POLICY: A place in a team cannot be guaranteed. If a player is not allocated to a team a full refund will be granted. Any other reason for a refund must be first discussed with the committee.
The Club does not participate in "Club Shopping" - especially for All Age Players. Players are expected to Register and Pay before they trial. Fees will be only refunded upon receipt of a written request providing a reason acceptable to the Executive Board of the Club. Any refund amount is calculated from the time your letter or email is submitted along with all written documentation required to process the refund.
If a request for refund is made within five (5) business days from registration for an U6 or U7 player, a full refund will be issued. In all other cases, when a refund is requested, the request will be considered by the Executive Board and MAY be paid minus a processing fee, as determined by the Board. The processing fee will be a minimum of $25, and if payments have been made to the KDSA, their processing fee will be included as well. All non-refundable components such as insurance and player fees paid to KDSA, Football NSW, or Football Federation Australia will not be refunded.
